Rate Re-Lock or Extension Form
Re-Lock: Must be done after rate expires. Pricing will be "worst case." A re-lock can be for 15 or 30 days, with 30 being the maximum. Request must be received prior to 3:30pm pacific time - requests received after 3:30pm will be subject to the next days pricing.
Extension: Must be done PRIOR TO 3:30pm pacific time of the commitment expiration date.

Any adjustment to your lock will require USA Direct Funding to re-issue the GFE to your borrower within 3 days.
